The Platinum Plan is our all-inclusive plan, extending our highest levels of service and access, and is designed for our patients who insist on a superior level of personalized care. With a further focus on prevention and wellness, this plan includes an annual executive physical exam and a nutritional consultation in addition to comprehensive primary medical care.  The Platinum Plan includes:

• All primary care office visits including routine care, urgent care, and an annual physical exam

• Routine injectables such as tetanus and flu vaccine
• Email & phone consultations
• Coordination of Specialty Care
• $600 laboratory allowance
• $400 radiology allowance

• House/Office calls when appropriate
