SILVER membership PLAN:  $100 per month

Our Silver Plan is suited for our patients with traditional insurance benefits but who desire an alternative to the traditional practice setting.

GOLD membership PLAN: $150 per month

Our Gold Plan is ideal for our patients who understand the ‘consumer direction’ that healthcare is taking and, accordingly, have high-deductible health insurance policies supplemented by Healthcare Savings Accounts (HSA). This plan is also ideal for those that carry no insurance at all. 

PLATINUM membership PLAN: $200 per month

The Platinum Plan is our all-inclusive plan, extending our highest levels of service and access, and is designed for our patients who insist on a superior level of personalized care. With a further focus on prevention and wellness, this plan includes an annual executive physical exam and a nutritional consultation in addition to comprehensive primary medical care.
